We provide care for the whole family, from routine checkups to more complex health and disability support. Whether you are managing a chronic health condition, navigating specialized care, or simply looking for a trusted clinic for you and your loved ones, we're here for you with compassion and expertise.
Services We Offer
While our clinic was initially created to serve individuals with disabilities and complex health needs, we now proudly welcome all individuals and families. For patients with standard care needs, we offer Primary Care services. Our primary care services are for children, teens, and adults and include wellness checkups, acute care (sick visits), chronic disease management, immunizations, and more. For patients with specialized care needs, we offer Complex Care services. Our complex care services are for children and adults with physical, developmental, or behavioral disabilities.

For Patients with Complex Care Needs
Our Complex Care Team provides quality health care services for patients with disabilities and conditions that include, but are not limited to, the following:
- ADHD (Attention-deficit hyperactivity disorder)
- Autism spectrum disorder
- Blindness or vision impairment
- Cerebral palsy
- Deafness or hearing impairment
- Down syndrome
- Epilepsy
- Stroke
- Traumatic brain injury

Complex Care: Adult and Pediatric Care Services
- Frequent wellness checkups and physicals, as needed
- Acute care (sick visits)
- Chronic disease management
- Immunizations
- Health and nutrition education
Complex Care: Women's Health Services
- Routine gynecological care and screenings
- Contraceptive and menstrual cycle management
- Management of gynecological problems, including minimially invasive gynecological surgery or procedures when medically necessary
- Prenatal and post-natal care
Complex Care: Behavioral and Mental Health Services
- Individual counseling for patients with disabilities and their family
- ADHD diagnosis and management for pediatric patients with disabilities
- Autism screening and referral for diagnostic testing
- Screening for behavioral and learning disorders

Medical Complexity Disclaimer: At the CommuniCare clinic at The MAC, the Complex Care department welcomes patients with all diagnoses and disabilities, but is currently only accepting those with low to moderate medical complexity. We are not able to serve patients with intensive medical needs such as patients who require the following:
- Hospice or palliative care
- Tracheostomy or home ventilator dependence
Appointments: A referral is required to be seen by our Complex Care Team.
