About Our Complex Care Program
At CommuniCare, we understand that navigating the healthcare landscape can feel overwhelming, especially for families caring for individuals with physical, developmental, or behavioral disabilities. That’s why we are proud to offer compassionate and expert health care services at The Morgan's MAC, located in northeast San Antonio.
We welcome individuals and families with complex care needs, from newborns to adults under 60 years old. That said, one of the best features of our program is that once you are enrolled with us, you never need to worry about aging out!
Who Qualifies for Complex Care?
- ADHD (Attention-deficit hyperactivity disorder)
- Autism spectrum disorder
- Blindness or vision impairment
- Cerebral palsy
- Deafness or hearing impairment
- Down syndrome
- Epilepsy
- Stroke
- Traumatic brain injury
Medical Complexity Disclaimer: At the CommuniCare clinic at The Morgan's MAC, the Complex Care department welcomes patients with all diagnoses and disabilities, but is currently only accepting those with low to moderate medical complexity. We are not able to serve patients with intensive medical needs such as patients who require the following:
- Hospice or palliative care
- Tracheostomy or home ventilator dependence
Appointments: A referral is required to be seen by our Complex Care Team.
Complex Care Services
Adult and Pediatric Primary Care Services
Acute Care
- Sick visits
- Basic infectious disease testing – Influenza, Strep, COVID-19
- Assessment of new problems with appropriate management and referrals for additional laboratory testing, imaging, and specialty care as needed
Preventative Care
- Annual wellness checkups
- Follow-up chronic care visits (every 3-6 months)
- Immunizations
- Routine screening evaluations based on age and underlying diagnoses
Women's Health Services
- Routine gynecological care and screening
- Contraceptive and menstrual cycle management
- Management of gynecological problems, including minimially invasive gynecological surgery or procedures when medically necessary
- Prenatal and post-natal care (obstetrics)
Behavioral and Mental Health Services
- Individual counseling for patients with disabilities and their family
- ADHD diagnosis and management for pediatric patients with disabilities
- Autism screening and referral for diagnostic testing
- Screening for behavioral and learning disorders
