For Pediatrics/Teen Health Appointments: You can make a pediatric primary care appointment for your child or teen using our online patient portal or by giving us a call.

For Adult Medicine Appointments: You can make an adult primary care appointment for yourself using our online patient portal or by giving us a call.

For Women's Health Appointments: To make an appointment with our OBGYN, Dr. Shawna Wall, please call us at (210)-233-7000 or come to our clinic in person to schedule. At this time, we do not offer online booking for Women's Health services.

If you wish to be seen or have your child be seen at our Helotes Clinic, but you do not have health insurance coverage, our Medical Eligibility Specialists (MES) are here to help. Before scheduling an appointment, you must speak with a MES team member; they will work with you to explore options like Medicaid, Medicare, or a Health Insurance Marketplace insurance plan.

If you don’t qualify for any of those plans, you may still be eligible for our Sliding Fee Discount Program, which reduces the cost of services based on your income and family size. Please call (210) 233-7076 or email us at mes.team@communicaresa.org to set up a call with a MES team member.

CommuniCare's Discount Program is an FQHC initiative that uses sliding scale fees to determine the percentage amount to be discounted from services provided to you as a patient with us. At CommuniCare, we use the sliding fee scale set by the Health and Human Services Commission.

To qualify for a sliding fee discount program, you must have a total family income that is less than 200% of the Federal Poverty Income Guidelines. To assist us in assessing your eligibility for the program, you will need to provide the following required documents at your initial member registration appointment: proof of identification, income, and residency.
