Schedule by Phone: Call us at (210) 233-7000 and ask to schedule a pediatric or adult medicine appointment at the Boerne Campus clinic. You will receive a courtesy call or text reminder for your appointment as well.

Parents of children with Medicaid and CHIP will need to provide a valid picture ID and the child’s Medicaid/CHIP card. If you wish to take advantage of services not covered by your child's plan, you can apply for our sliding fee discount program. We accept the following Medicaid/CHIP Plans: Community First Health Plans, Superior Health Plan, and Medicaid Traditional.

For patients with Medicare or Medicare Advantage, you will need to provide a picture ID and your Medicare card. We accept the following Medicare/Medicare Advantage Plans: Aetna, Allwell/Superior HealthPlan, BCBSTX, Community First, HealthSpring, Humana Gold, Medicare NGS, United/WellMed, United Healthcare, and Wellcare.

If you wish to become a patient at our CommuniCare Boerne Campus clinic and do not have health insurance, you must first visit with a CommuniCare Medical Eligibility Specialist prior to your appointment. They can help you navigate the Health Insurance Marketplace to find a plan that best suits you and your family's needs, or see if you qualify for programs like Medicaid, Medicare, or the sliding fee discount program.
