How To Become A Patient FAQs
Schedule Online: You can schedule a pediatric medical appointment online.
Schedule by Phone: Call us at (512) 268-8900 to schedule by phone. You will receive a courtesy call reminder for your appointment as well.
If you have private/commercial health insurance, you will need to bring your insurance card and a picture ID to your child's appointment. If you do not have a current insurance card, you can provide a letter from your insurance company that contains information about your healthcare coverage.

We accept the following Medicaid/CHIP plans: Blue Cross Blue Shield, Community First, Superior, and Medicaid Traditional. Parents of children with Medicaid and CHIP will need to provide the child’s Medicaid/CHIP card and a picture ID at the appointment.
If you wish to take advantage of services not covered by your health insurance plan, you may apply for our Discount Program, which reduces the cost of services based on your income and family size.
The CommuniCare Discount Program is an FQHC initiative that uses sliding scale fees to determine the percentage amount to be discounted for our healthcare services. At CommuniCare, we use the sliding fee scale set by the Health and Human Services Commission. To qualify for the discount program, you must have a total family income that is less than 200% of the Federal Poverty Income Guidelines. To assist us in assessing your eligibility for the program, you will need to provide the following required documents at your initial member registration appointment: proof of identification, income, and residency.
