Under the Health Insurance Portability and Accountability Act (HIPAA) of 1996, you have the right to access, inspect and/or obtain a copy of your protected health information (PHI). At CommuniCare, there are several ways you can request your medical record.

You can access immunization records, test and lab results, medication lists, after-visit summaries and notes through Healow. You can share and print copies of this information for free at any time.

You can request your medical record online by submitting the authorization request form on a personal computer or mobile device.

You also have the option to request your medical record by mailing in a printed copy of the authorization form. You can submit the form by physical mail, fax, or email.
