Parents of children with Medicaid and CHIP will need to provide a valid picture ID and the child’s Medicaid/CHIP card. If they wish to take advantage of services not covered by their plan, they can apply for our Discount Program.

We accept the following Medicaid/CHIP Plans: Community First Health Plans, Superior Health Plan, and Medicaid Traditional.

If you wish to become a patient at our CommuniCare Boerne Campus clinic location and do not have health insurance coverage, you must first visit with a Member Services Representative prior to your appointment, who can assist you with the process of applying for our sliding fee discount program.
