Patients with Medicaid, CHIP, or Medicare will need to provide a valid picture ID and the patient’s Medicaid/CHIP or Medicare card. If they wish to take advantage of services not covered by their plan, they can apply for our “discount program.”

We accept the following Medicaid/CHIP and Medicare Plans: DentaQuest Medicaid/CHIP, MCNA Medicaid/CHIP, and Envolve Allwell Medicare Advantage.

If you wish to become a patient at our CommuniCare BLVD Dental location and do not have health insurance coverage, you must first visit with a Member Services Representative prior to your appointment, who can assist you with the process of applying for a discount program.
