Schedule by Phone: Call (210) 233-7000 to schedule by phone. You will receive a courtesy call or text reminder for your appointment as well.

Patients who are covered by Medicaid, CHIP, or Medicare are required to provide a valid photo ID along with their Medicaid/CHIP or Medicare card. Patients who want to use services not covered by their plan can apply for CommuniCare's "Discount Program."

We accept Medicaid/CHIP Plans from Community First, Superior HealthPlan, and Medicaid Traditional. We accept Medicare Plans from Aetna, Allwell/Superior Health Plan, Blue Cross Blue Shield, Community First, HealthSpring/WellMed, Humana Gold/WellMed, Medicare Traditional, United/WellMed, and Wellcare.

The CommuniCare Discount Program is an FQHC initiative that uses sliding scale fees to determine the percentage amount to be discounted for our healthcare services. At CommuniCare, we use the sliding fee scale set by the Health and Human Services Commission. To qualify for the discount program, you must have a total family income that is less than 200% of the Federal Poverty Income Guidelines. To assist us in assessing your eligibility for the program, you will need to provide the following required documents at your initial member registration appointment: proof of identification, income, and residency.
